Issue:
On 04/04/12, Veteran A contacted the Release of Information (ROI) office, Nashville Campus, to report he received medical records in the mail (approximately 100 pages) which belonged to Veteran B. Upon investigation, the Supervisor of the ROI office found Veteran Bs records were inadvertently mailed to Veteran A by an ROI clerk. The Supervisor spoke with Veteran A by phone and made arrangements for Veteran Bs records to be returned to the ROI office via a self-addressed UPS envelope.Personal identifiable Information (PII) includes: Other than the contact information, it is unknown. Update: 04/05/12:Veteran B will be sent a letter offering credit protection services due to full name, SSN and medical information being exposed.
Outcome:
4/17/12: ROI clerk received written counseling. PO mailed CM Letter.
