Issue:
Police service discovered several documents containing personally identifiable information (PII)/protected health information (PHI) in a trash can in front of the main entrance of our facility. The PII/PHI were from the University of Miami, Jackson Health System, and the Miami VA. Out of the several documents, 6 pages were from the VA (the majority were from the other institutions) and contained full SSNs for 23 VA patients and 7332 information. It appears that we were able to trace the documents back to one resident. Update: 05/02/13:Letters will be sent to 23 patients offering credit protection services.05/29/13:This was determined to be HITECH reportable by VHA Privacy Office.
