Reported Entity: VISN 11 Battle Creek, MI

Issue:

An excel spreadsheet was discovered to be missing by a Physical Therapy Assistant at the outpatient clinic. The employee states he had it one minute, provided treatment to a Veteran and the sheet was gone. A thorough search did not locate the sheet. Update: 10/11/11:Waiting on counts of unique Veterans/Patients affected by loss.10/16/11:Informed by supervisor the appointment list from VISTA is the missing document, not the excel spreadsheet. This impacts the information that has been lost, to include full SSN, date of birth, full name, home phone number and co-pay status. Still waiting on counts.10/17/11:The number of unique Veterans was 23, therefore 23 Veterans will receive a letter offering credit protection services.

Outcome:

The employee has been counseled, and the clinic is not going to print or use the VISTA appointment list anymore. Credit Monitoring letters have been sent to 23 impacted Veterans. VISTA appointment list will no longer be printed or used by GROPC - PT staff.
