Issue:

On 10/25/11, a Life Insurance company faxed a request and signed release from Veteran A for the last 5 years of medical records. Veteran A then revoked the release in a letter that was faxed to the VA two days later on 10/27/11. However, the Insurance company re-faxed a second request for records on 11/09/11 and the Release Of Information (ROI) clerk inadvertantly processed the request and sent all the records for the last 5 years to the Life Insurance company. Update: 12/07/11:Veteran A will be sent a notification letter.

Outcome:

Supervisor and Manager re-educated employee.
