Issue:
Patient A received a prescription intended for Patient B. Patient Bs name and type of medication was compromised. Patient A reported the incident to the medical center and a replacement has been requested for Patient B. Dallas Consolidated Mail Outpatient Pharmacy (CMOP) investigation concludes that this was a CMOP packing error. The packer unknowingly left one medication for Patient B out of the package. As he completed the packing of the next order he noticed Patient B medication and assumed it belonged in the package just completed for Patient A. The packer requested a manager reprint the packing label. Neither the manager nor the packer rechecked the package to insure the contents were correct. This procedure should have been done based on established policy. The CMOP employees will be counseled and retrained in proper packing procedures. Update: 06/28/11:Patient B will be sent a notification letter.NOTE: There were a total of 1 Mis-Mailed CMOP incidents out of # total packages (# total prescriptions) mailed out for this reporting period. Because of repetition, the other 0 are not included in this report, but are included in the "Mis-Mailed CMOP Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter.
Outcome:
The CMOP employees were counseled and retrained in proper packing procedures.
