A VA employee in the Release of Information (ROI) Office faxed the records of Veteran A to Veteran B; Veteran B returned the mis-faxed records to this facility. The information disclosed included Veteran A's name, address, full SSN, date of birth and protected health information (PHI). Update: 04/11/12:Veteran A will be sent a letter offering credit protection services due to full name and full SSN being exposed.
Outcome:
The employee that faxed the information will be inserviced and counseled by the Supervisor.
