There was a mismailing from the VA San Diego Healthcare System. A mailing was sent to Veteran A and was reported to the Northern Arizona Health Care System Privacy Officer by Veteran B who received the information. The data was secured by the Privacy Officer (PO). Update: 05/09/12:Veteran A will be sent a letter offering credit protection services due to full name and full SSN being exposed.
