Issue:

On 07/27/12, Veteran A from the Abilene Community Based Outpatient Clininc (CBOC) dropped off 2 packages that were delivered to his house with his address on the package and the Veteran B's name sent from VHA 18 Pharmacy (WTVAHCS). Out of concern Veteran A turned in the package which contained medication, he also recieved an envelope from Eligability requesting his spouses 10/10EZ. Veteran B is from the Odessa area. Veteran A that recieved the packages did not open the packages he delivered them immediately to the VA CBOC nearest him (Abilene CBOC). Update: 07/29/12:Veteran A will receive a letter of notification..

Outcome:

Health Administration Services staff corrected the Veterans name and addresses . Pharmacy was also contacted to verify and correct medication distribution. Veterans have been contacted for verification and update of record information and notification of mix up.
