Issue:

Medical record documents of an active duty spouse were milsfiled in the record of another active duty member. the documents contained the spouse's name, full SSN, date of birth and protected health information (PHI). Update: 04/13/12:An active duty military member appeared for an appointment with his records. Upon arrival at the Medical Center, he noticed copies medical records belonging to a military dependant filed in his records. The military member contacted the dependants sponsor to return the records. The military dependant will be sent a letter offering credit protection services.Medical records of military dependants also contain the name and SSN of their sponsor. The dependants spouse will also be sent a letter offering credit protection services.

Outcome:

Supervisors of the DoD and FHCC file rooms conducted refresher instruction to their respective file clerks. Credit monitoring letters were prepared and mailed.
