Issue:

Based on record review, the hospital failed to safeguard Patient 1's Protected Health Information (PHI) which resulted in an unauthorized person receiving his PHI. Findings:During review of the facility Unusual Occurrence Report dated 4/23/13, the report indicated RN 1 (Registered Nurse) placed Patient 1's private health information into another patient's discharge papers. The private health information consisted of Patient 1's name and medical record number. RN 1 received counseling and and was required to complete further privacy training.
