Issue:

Two Veterans were in the hospital on the same day and a radiology report for Veteran B was given to Veteran A at discharge. The Veteran's wife contacted telephone triage to ask some questions and it was found out then that they had a report that did not belong to them. The original report is now with the Privacy Officer. Update: 10/19/12:Due to full SSN and medical information being exposed, Veteran B will be sent a letter offering credit protection services.

Outcome:

The supervisor has instructed all staff on the following:Please make sure that when you are pulling items off the printer that you check each paper you pick up to make sure it is for the Veteran you requested an item to print for. Also if you are printing several items for a project, check every page to make sure that an order page has not printed along with your print request.When you are handing a Veteran their discharge instructions, make sure that each page you hand them is for them.
