Issue:

A DOM Technician allegedly provided DOM Resident A with the roster to show him his eligibility for DOM privileges (or lack thereof). The technician failed to secure it and Resident A retained the roster until yesterday, when Resident B (during therapy session) mentioned that Resident A had it in his possession. When asked, Resident A surrendered the roster and stated that no copies had been made (no access to copy machine), and the roster had not left campus. Reports of Contact (ROC) from Residents A & B, and the DOM Tech are being collected. The roster had the residents' first, last, middle initial, last four SSN, age, admission date, total days at the DOM, and the name of the primary physician. As a precaution, the PO is assembling the facility's Incident Response Team (IRT) for further review. Update: 01/12/11:Upon examination of the DOM roster it shows 55 names, therefore fifty-five (55) patients will receive a notification letter.

Outcome:

The importance of protecting private information of residents who are served in the program was discussed at the Dom Morning Report meeting In addition, the staff is engaged in discussions of observing for areas where private information might be at risk for a breach. During the weekly scheduled Team-Building Meetings, information security and privacy will be one of the major agenda items to further inform and educate Dom staff..
