Issue:
The Release of Information Office received a package from the USPS which contained medical documents that were found loose in the mail or damaged by mechanical processing equipment. The original package that was sent form Release of Information via certified mail contained approximately 600 pages, the package that was returned from the USPS contained 200 pages. The ROI staff contacted USPS and reported that 400 pages were missing. The USPS entered a claim and provided a claim number to ROI. At this point 400 pages are unaccounted for. Update: 02/14/13:The missing records only pertain to one Veteran. This was a routine request from a Veteran. Although this was not the fault of the VA, the Veteran will be sent a letter offering credit protection services.
Outcome:
A claim was submitted to the USPS, and we will be contacted if the missing records are found at some point.
