Issue:

An employee health chart was left at the front desk of the hospital for 5 days. This chart was behind the desk and there was always an employee in attendance, however, some employees who did not have a need to know may have had access to the chart. There is a paper attached to the front of the chart that contains an employee's full name & full SSN as well as a patient's last name and last four. Update: 05/15/12:One employee will be sent a letter offering credit protection services due to full name and SSN being displayed on the front of the chart.NOTE: There were a total of 33 Mis-Handling incidents this reporting period. Because of repetition, the other 32 are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.

Outcome:

Process for tracking employee health charts will be examined and updated to avoid similar situations in the future. Staff reminded that these records are to be returned to proper office as soon as possible.
