Issue:

Veteran A notified the Community-Based Outpatient Clinics (CBOC) that he received Veteran B's mail. The Privacy Officer (PO) contacted Veteran A who reported that he received 4 pages of Veteran B's appointment letters. Documentation on letters reviewed and contain appointment information, including clinic name. Veteran A lives alone 50 plus miles from nearest CBOC. Veteran ensures 4 pages are secured and will return documents to CBOC next week during their clinic appointment. Update: 02/10/12:Veteran B will receive a letter of notification since the appointment letters contain appointment information, which is protected health informaiton (PHI).

Outcome:

Facility awaiting return of documentation from Veteran A. Service area unable to determine if incident occurred as a result of human or machine error.
