Issue:

A military dependent received a one page document containing medical information on an active duty member. the information on the active duty member included the name, full SSN and diagnosis. Update: 09/19/12:A military dependent requested her records from Release of Information (ROI). She did not view them until sometime later. When she did, she found one page of a consultation report on an active duty member mixed in with her documents. She brought the document in and reported it.When a request for records comes to the ROI Unit, the clerks have to print the documents from both CPRS and AHLTA. CPRS queues the documents. AHLTA does not. This means that if CPRS is printing and another clerk prints documents from AHLTA, the AHLTA print could get printed in the CPRS document batch. Before releasing the documents to the requester, the ROI clerks have to look through the documents to make sure an AHLTA print job is not included. This one page document was missed.The ROI clerks have been assigned specific printers to print to, in an effort to avoid this from happening in the future. 09/20/12:The active duty member will receive a letter offering credit protection services.

Outcome:

Employees educated and policies have been changed.
