Issue:

Third party medical provider contacted Privacy Officer to notify that a patient who receives care at the VA and the third party facility. The VA provider or administrative staff for the provider mailed a patient documents from three other patients, all containing full name, address and medical information. One contained full SSNs as well. The third party provider mailed all documents back to the Privacy Officer. Update: 12/20/11:One (1) Veteran will be sent letters offering credit protection services due to full SSN being disclosed. The other two Veterans will received notification letters due to PHI being disclosed.

Outcome:

Employee retook the training, was counseled and advised to revise procedures to prevent reoccurrence of issue.
