Issue:

On 02/25/15, Veteran As medication was picked up from the Pharmacy Window by an authorized family member. Later on that same day, Veteran A recognized that he also received the medications of Veteran B and contacted the Facility to report the error. On the afternoon of 02/25/15, the relative of Veteran A returned the medications of Veteran B to the Pharmacy. The protected health information (PHI) of Veteran B that was involved included his full name, address and the names of the medications.

Outcome:

02/27/15: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
