Issue:
Community Based Outpatient Clinic (CBOC) Administrative Officer (AO) received a call from a Avis Car Rental Manager in Conyers, GA saying that they received a fax containing a Veteran's personal information. He stated that he stopped the fax from printing after 7 pages. He said that he had a secure shred bin and was going to shred the information. The individual who faxed the information was not given the correct fax number as the AO verified the fax number given with the two individuals involved. Update: 10/20/11: One Veteran will be sent a letter offering credit protection services.
Outcome:
Employee failed to verify fax number. The number was taken directly from providers notes. Remedial training provided by supervisor.
