Issue:

Based on staff interview and record review, the facility failed to maintain the privacy and confidentiality for one patient (Patient A), when an employee gave discharge instructions for Patient A to another patient.Findings:An interview was conducted on 1/10/12 at 10:00 AM, with the Health information Officer (HIO). The HIO stated that on 12/16/11, an employee was discharging Patient A. The HIO stated that the discharge papers had to be picked up in another unit because the printer was not working in the employees unit. The HIO stated that when the employee went to the other unit to retrieve Patient A ' s discharge instructions, the employee inadvertently picked up discharge instructions that belonged to another patient with the same last name.Review of the discharge paperwork disclosed included the following PHI: Patient name Diagnosis Age Sex Date of birth Physician Discharge instructionsThe HIO further stated that the patient was notified to disclose that personal information was given to someone else inadvertently. The HIO stated that Patient A brought the information back to the facility when noticed that the discharge instructions had someone else ' s name on them.
