Issue:

On April 27, 2015, Veteran A went to the VA Tullahoma Outpatient Clinic to report and to turn in documentation he received in the mail that did not belong to him. Although the cover page did contain Veteran A's name and address, in that packet was also VA Form 10-3542, Veteran/Beneficiary Claim for Reimbursement of Travel Expenses, which belonged to Veteran B. Veteran A turned in all of the documents he received to the Clinic, who in turn provided them to the Privacy Officer. PHI of Veteran B that was disclosed included full name, full SSN, date of birth and address.

Outcome:

04/27/15: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
