Issue:

Veteran A contacted the Chattanooga CBOC and reported she received an appointment letter which belonged to Veteran B. Veteran A stated she destroyed Veteran Bs letter via shredder. Veteran Bs personal identifiable information that was disclosed included: full name, last four SSN, and address. The Privacy Officer, Nashville Campus, spoke with the Clerk, Chattanooga CBOC, who stated they are unable to determine who is responsible for the error. Chattanooga CBOC hand-folds these letters and sends out hundreds a week.

Outcome:

05/16/13: Veteran B will be sent a notification letter. 06/05/13: This was determined to be HITECH reportable by VHA Privacy Office.
