Issue:

Veteran A was handed a packet containing information for Veteran B. The packet contained the name, full SSN, the spouses name, address, telephone numbers, other personal information, and a list of medications. Veteran A was looking at it later and realized the medications were not his. He turned the list into the patient advocates office. Update: 01/28/13:Veteran B will be sent a letter offering credit protection services.NOTE: There were a total of 23 Mis-Handling incidents this reporting period. Because of repetition, the other 22 are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.

Outcome:

Additional training was provided to staff to avoid incidents like this in the future.
