Issue:

The printed progress notes of two (2) Veterans were found on a printer in unlocked, common area. The time stamp on papers say that it was printed 12/11/12 and 12/12/12. The documents were found by the IT specialist when the printer was moved into Warehouse on LSCVAMC Campus, also a crowded common area. The IT specialist turned these docs in to the Facility Information Security Officer (FISO). The FISO notified the LSCVAMC Privacy Officers via encrypted Outlook email. The progress notes contained the veterans' name, full SSN, date of birth, and protected health information (PHI). Update: 01/23/13:The two Veterans will receive a letter offering credit protection services.

Outcome:

Credit Monitoring letters mailed on 1-30-13. PO performed walk through of Warehouse to ensure no additional data was found.
