Issue:
Outpatient Pharmacy Supervisor notified Privacy Officer a shipping error occurred where two patients meds were mailed incorrectly. Only one had been delivered and the second was intercepted through UPS. Patient A received a prescription through UPS overnight for Patient B. Patient A returned Patient B's prescription the same day he received them. No SSN infotmation was given out but the drug name and Patients address was on the prescription. The process was reviewed and it was determined that the error occurred in the temporary basement vault area when the prescription was wrapped for mailing. The Outpatient Pharmacy Supervisor reported that more counter space will be added to the area and Pharmacists will assist in the mail wrapping process to help prevent this from occurring in the future. Update: 11/21/11:Patient B will receive a letter of notification.
