Issue:

Physician A inappropriately accessed Physician B's medical record. Physician B was not a patient and had not received treatment from Physician A at any time. Update: 03/28/11:The medical record included Physician B's name, full SSN, date of birth, and full medical information. Physician B will be offered credit protection services.NOTE: There were a total of 16 Mis-Handling incidents this reporting period. Because of repetition, the other 15 are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.

Outcome:

This investigation is completed and closed. The recommendation was disciplinary action. Physician A was counseled and will receive a suspension to be determined by Service Chief and Human Resources.
