Issue:

PO was notified that medications and education sheets were intended for one Veteran and received by another by mail. On February 18, 2011, Veteran A returned medication and education sheets meant for Veteran B to CBOC Pharmacy. No medications had been taken in error by Veteran A, returning PHI and medications intact.Medication was caffeine/ergotamine. Correct medications were dispensed to both patients immediately after Pharmacy event triage. Returned PHI and meds discarded per protocol upon Pharmacy receipt 02/18/11. Appropriate notifications and investigation to ensue. Update: 02/22/11:Veteran B will receive a letter of notification.

Outcome:

ROC written by employee involved in providing medication to another Veteran in error. Re-education by Pharmacy Ass't Chief and performance improvement review of medication dispensing in this CBOC.
