LETTER OFFERING CREDIT MONITORING WAS MAILED TO THE VETERAN ON 4/19/13. THE CLINIC ADMINISTRATOR (CBOC) WAS NOTIFIED. EDUCATION WAS PROVIDED TO THE APPROPRIATE STAFF.
