Issue:
Release of Information (ROI) employee received an envelope of partial medical records of a patient. There was a sticker on the envelope from the Postal Service that says 'we're sorry that your article was damaged during processing'. The information returned was pages 242 through approximately 304. Pages 1-241 are not in the envelope and we do not know what happened to the rest of the documents. Update: 06/06/12:Although not the fault of the VA, one Veteran will be sent a letter offering credit protection services due to full name and SSN being disclosed.
Outcome:
No fault of the VA.
