Issue:

Patient A's package was sent to an incorrect address. Patient As name, address, and type of medication was compromised. The VAMC reported the incident to CMOP and a replacement has been requested for Patient A. Great Lakes Consolidated Mail Outpatient Pharmacy (CMOP) investigation concludes that this is the result of a VAMC incorrect address transmission leading to a USPS misdelivery, not a CMOP packaging error. The medical center has been notified. Update: 07/24/12:Patient A will be sent a notification letter.
