A male VA employee was allegedly dating or wanted to date a female patient. He allegedly accessed the patient's records (outside his normal duty hours) which leads his supervisor to believe he had no need to know since he was not on duty.

Outcome:

01/14/15: The Incident Resolution Service Team has determined that one Patient will be sent a HIPAA notification letter due to Protected Health Information (PHI) being accessed inappropriately.
