Veteran A reported that he received Veterans B's "Department of Motor Vehicles Driver Medical Evaluation" and his VA form 10-5345 "Request for and Authorization to Release Medical Information or Health Information" that contained Veteran B's full name, full SSN, DOB, Drivers License number, home address, phone number, medical information and medications.
Outcome:
07/17/14: The Incident Resolution Service Team determined that Veteran B will receive a letter offering credit protection services.
