Issue:
A VA employee was assisting a Veteran who placed his folder on top of a paper that an employee was working on and when the Veteran took his folder he picked up the paper as well (Veteran B's 10-10 EZ). Veteran A who picked up the form returned it on Monday. The incident occurred on a Friday. The VA freestanding ambulatory care center is closed on weekends. Veteran A had Veteran B's information which included his full name, full SSN, DOB, employment, and address. Update: 01/10/12:Veteran B will be sent a letter offering credit protection services.NOTE: There were a total of 24 Mis-Handling incidents this reporting period. Because of repetition, the other 23 are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.
Outcome:
The supervisor of the employee was contacted and asked to instruct the employee (and others) not to keep working papers on counters or desks when other patients come to them for assistance and to raise awareness with this example.
