Issue:
A Veteran received call from her estranged father on her cell phone. Her number had been changed and it had not been given to him. He stated to her he was going to attack her at home after her surgery. No one knew she was scheduled for surgery. The Information Security Officer ran a Sensitive Patient Access Report and found multiple access by Department of Defense. The Veteran is not on active duty nor has been to any DOD facility in several years. The issue has been reported to the OIG. Update: 05/22/13:The Veteran will be sent a HIPAA notification letter due to Protected Health Information (PHI) being accessed inappropriately..
