Issue:

The Provider, ED, Murfreesboro Campus, was given peer review documents to review. The Provider put the documents in a red folder and placed the folder on his desk which is located behind the nurses station. The Provider became distracted and left the area. When the Provider returned, the red folder was missing. The Provider searched the area, questioned other staff regarding the missing documents, but was unable to locate the folder. The Provider stated the documents contained the full name, last four social security number, and peer review findings on four (4) Veterans. The peer review findings did not contain medical information only a rating regarding the level of care provided to the Veterans.

Outcome:

09/24/14: The Incident Resolution Service Team has determined that four Veterans will be sent a notification letter. 10/01/14: One of the Veterans is deceased so their next of kin will be sent a letter of notification.
