Issue:

On February 3, 2014 the Pharmacy Supervisor reports to the Privacy Officer that a controlled medication was given to Veteran B instead of Veteran A in the Emergency Department on 2/2/2014. Both patients have a similar sounding last name. Veteran B responded to the medication call for Veteran A, taking the medication and accompanying paperwork then departing the facility. Later, Veteran A requested his medication which lead to the discovery of the transposed medications. Veteran As correct medications were refilled and dispensed. Several messages have been left for Veteran B to recover medications and materials. Additional notification and investigation, in progress.

Outcome:

02/03/14: Veteran A will be sent a notification letter.
