Issue:

A Physicians Health & Injury Clinic employee contacted the Dorn VA Medical Center Release of Information (ROI) to inform her that she faxed a request for copies of radiology reports for Veteran A early this morning. She advised that the copies of medical records faxed to her were intended for another Veteran. No 7332 information was released. The Physicians Health & Injury employee stated that she shredded the documents. The correct records on Veteran A were faxed as requested. The information disclosed: Fourteen radiology reports, Veteran B full name, address, full social security number, and the providers name. The error occurred on 6/20/2012. Update: 06/21/12:One Veteran will be sent a letter offering credit protection services.

Outcome:

Employee counseled on 6/20/12. Privacy incident reviewed with all staff at HIMS staff meeting on 7/10/12.
