Issue:

On 05/16/12, the Privacy Officer (PO), Nashville Campus, was notified of the following incident via an anonymous note left outside her door. A Veterans documents were attached to the note.The note indicated documents containing a Veterans personal health information (PHI) were found in the ladies restroom (3rd floor, Nashville Campus). The Veterans PHI included a consultation sheet and instructions from the Wheelchair Specialty Clinic. Veterans PHI contained on the documents included: Full name, full social security number, date of birth, and diagnoses.Upon inquiry, the PO learned the Veteran had an appointment in the Wheelchair clinic that day. The PO learned the documents were inadvertently left in the restroom by the provider who treated Veteran during the appointment. Update: 05/17/12:One Veteran will be sent a letter offering credit protection services.

Outcome:

5/24/12 - Incident due to a human error. First incident for this employee. Employee educated on the importance of safeguarding Veteran PII. CM letter mailed.
