Based on interview and record review, the facility failed to protect the privacy of personnel health information (PHI) for one patient (Patient A) , when a "Letter of Medical Necessity", which was to be emailed to him, was sent to the wrong email address resulting in a breach.FindingsOn 5/17/12 at 9:30 AM, an investigation was initiated regarding a facility self report of a possible breach of Patient A's PHI.During a review of a letter on 5/11/12, the corporate compliance department staff had written to Patient A to tell him that some of his PHI may have been inadvertently sent to the wrong person.During an interview with the corporate compliance staff on 5/17/12 at 10:30 AM, she stated ,"[used name of hospital department] secretary was given an email address by the registered nurse practioner (RNP)for Patient A. The secretary was to email Patient A's "Letter of Medical Necessity" to him, so that he could contact his insurance company. The RNP gave the secretary Patient B's email address inadvertently. It was discovered during quality monitoring". The information included Patient A's name, date of birth and diagnosis.
