Issue:

A Veteran called to report she received 3 other Veterans' test results in the mail. The documents included the Veterans' names, partial SSNs and test results. The Privacy Officer (PO) spoke with the Veteran and her daughter who stated they called the VA and were told to shred the documents. They shredded the documents as soon as they spoke with someone at VA and are planning to burn the shredded material as soon as possible. Update: 10/28/11:The three Veterans will receive a letter of notification.

Outcome:

Secretary who sent letters by mistake was counseled by supervisor and educated regarding the protection of Veteran privacy.
