Issue:

On October 21, 2013, an Employee/Veteran from the VA Chattanooga CBOC requested and received a Sensitive Patient Access Report (SPAR) to be able to view who had accessed his CPRS/VISTA record. This Employee/Veteran, upon receiving and reviewing the SPAR, notified this Privacy Officer of two suspicious entries that he wanted to be checked to determine if their access was justified.

Outcome:

10/28/13: PO is still investigating accesses; awaiting statements. 10/30/13: PO received statement from employee (Nurse) where access could not be justified. The PO reviewed this Veterans/Employee's CPRS record thoroughly to try to determine why this Nurse may have accessed on 2/19/13 and PO could not find a reason for accession. This Nurse stated she works with various Providers and may have needed to access for her duties, although this could not be justified. 10/31/13: Employee/Veteran A will be sent a notification letter.
