Issue:

Employee A brought 2 print jobs (job was printed twice) that was found on a printer (SURG6) In reviewing the print job, there are a total of 9 patients information listed, to include PHI. The email was printed from a HOTMAIL acocunt that had been forwarded from a va.gov account. Update: 09/17/12:Nine patients will be sent a letter of notification.10/24/12:This incident should not be assigned to Columbus. This incident occurred in the Miami VA and involved Miami VA employees. A Miami employee inappropriately selected/mapped to a Columbus printer. Our ISO entered it as belonging to Miami VA and for some reason it is now assigned to us. These patients are not in our system...since they belong to Miami...and we cannot send letters to them (no demographic infor) nor can we take any action with the employees involved in this incident since they are not Columbus employees. Our sole involvement was that one of our staff removed the document from the printer and took it to the ISO.

Outcome:

VA has taken specific administrative steps to ensure that this does not happen again and reminded staff of the importance of safeguarding PII and PHI.
