Issue:

An Appointment clerk handed Veteran A's appointment list to Veteran B. Veteran B discovered the error after arriving home and notified Telecare, who in turn corrected the appointments and urged the Veteran to return the appointment list for Veteran A. Veteran B mailed the list to the Privacy Officer. The Service Chief has been notified of the error and is checking for causal factors. Update: 07/09/12:Veteran A will be sent a letter offering credit protection services.NOTE: There were a total of 18 Mis-Handling incidents this reporting period. Because of repetition, the other 17 are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.

Outcome:

The clerk involved has been counseled to carefully cross-check all correspondence handed to patients. The appointment list was returned to the Privacy Officer.
