Issue:

10/3/2011, Veteran A arrived at the Dorn VA Outpatient Pharmacy window, because he had questions about a medication listed on his medication sheet. The Veteran presented the paperwork to the Pharmacy Technician at the window. Upon review of the paperwork, the Pharmacy Technician found that some of the paperwork was for a different Veteran. The Pharmacy Technician separated the paperwork and returned only the paperwork intended for Veteran A. The Veteran stated that he had received the paperwork from the Pain Clinic. The Privacy Officer was notified on 10/7/2011. The information disclosed: Veteran B full name, list of medications, last four of social security number, and information on how to obtain refills. When: The Dorn VA Medical Center Outpatient Pharmacy received the documents on 10/3/2011. Update: 10/11/11:Veteran B will be sent a notification letter.

Outcome:

Staff is being re-educated on the need to safeguard patient information.
