Issue:

Patient A requested a copy of his EMG report. The Release of Information (ROI) clerk printed out the report from the electronic medical record and mailed it to the patient on July 10, 2012. The ROI supervisor received a call from patient A who stated that he received another patient information. The patient was instructed to bring the documents back to the facility which he did today 7/16/2012. The issue is an employee in the Rehab department attached patient B EMG report to patient A medical record. The ROI clerk printed the report and did not see it that the name on it was incorrect mailed it out. Update: 07/16/12:One Veteran will be sent a letter offering credit protection services.

Outcome:

The PO educated the employee on how to check documents for accuracy before sending them to the requester. The employee was reminded of the process that was put in place to prevent this type of incident from happening. Disciplinary actions was also proposed for the employee.
