Issue:

A list used by Residents containing a patient's full name, last four digits of the SSN, age, diagnosis, ward and room number was found by a shuttle stop that transport resident from our affiliate to the VA. The list was found by the Alternate Privacy Officer (PO) on her way home at about 5:30 PM on 12/12/12. The list was given to the PO on 12/13/12. The PO spoke to the Resident whose name appeared on the list and he stated it was his but he does not know how it got out side. He stated that he left it at the nursing station. Update: 12/13/12:A letter of notification will be sent to one patient.

Outcome:

The resident was counseled regarding the seriousness of losing any document containing PHI especially during transport throughout the facility. He was also reminded that protecting sensitive information is the responsibility of all employees.
