Issue:
A Veteran went to the release of information department (ROI) and completed a release form. She was told that the records would be mailed. Another Veteran received his records at the ROI counter right after the previous Veteran. When he got home he noticed Veteran A's medical records mixed in with his own. He mailed the records to the other Veteran. She brought in the envelope showing he had sent her the records by priority mail. She made the complaint. Update: 11/22/11:Veteran A will be sent a letter offering credit protection services due to full name and full SSN being disclosed.
Outcome:
Provider has been educated and was required to re-take the Privacy Trng.Memo to HR sent.Ltr uploaded, closing ticket.
