A Veteran in outpatient lodger status found a Lodger checklist document and reported it to a nurse on 5-27-12. The document contained full names, partial SSN of 37 Veterans, which included medication information of 5 of the 37 Veterans. The list was dated 5-21 thru 5-25. The document was secured by the nurse on 5-27-12 and provided to the Privacy Officer at 7:50 am 5-29-12. Update: 05/29/12:Thirty-seven (37) Veterans will be sent a notification letter due to PHI being exposed.
