Issue:
A notification letter for an appointment for Patient A was sent to Patient B. The letter contained the name, last four digits of the SSN and address of Patient A, as well as his appointment information. Patient B notified the phone unit about receiving it. The Privacy Officer (PO) will call Patient B back to try to retrieve the letter. Update: 08/08/11:Patient A will be sent a notification letter.NOTE: There were a total of 17 Mis-Mailed incidents this reporting period. Because of repetition, the other 16 are not included in this report, but are included in the "Mis-Mailed Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.
Outcome:
Staff was reminded to use caution when handling documents with personally identifiable information (PII). The PO was unable to retrieve the letter.
