Issue:

VA employee reported to Privacy Officer he found inpatient food order slips and hand off sheets unattended on food carts. Privacy Officer went to check other areas and found additional inpatient food order slips and patient arm band on unattended food carts. Update: 07/25/11:Due to the extended time frame and the fact that there was 7332 information exposed, 20 Veterans will receive a letter offering credit protection services.01/24/12:It was reported that 5 of the Veterans are deceased, therefore 15 will receive a letter offering credit protection services.

Outcome:

Information was retrieved. Local policy and procedures were established for proper patient information on printed tray tickets and labels for delivering food to patients. Implemented additional training for the staff regarding handling personally identifiable and sensitive information.
